HOW TO PREPARE FOR YOUR MILITARY PERIODIC HEALTH ASSESSMENT (PHA)

To protect your individual medical readiness status and ensure your "IMR Green" clearance is updated quickly, please review these specific preparation instructions. Failure to complete your online requirements prior to your visit will prevent you from being seen.



???? STEP 1: Complete Your Online Questionnaire (Mandatory)

Before arriving at the clinic, you must complete the electronic portion of your PHA. Your clinical examiner cannot sign off on your medical readiness until this is finalized in the system.

·       Service Portals: Log into your respective branch health readiness portal (e.g., MEDPROS for the Army, ASIMS/MyIMR for the Air Force, or MRRS for the Navy/Coast Guard).

·       Action: Complete the entire online Periodic Health Assessment Questionnaire (PHAQ).

·       Print Confirmation: Once submitted, print the confirmation page or your PHAQ summary and bring a physical copy to your appointment. 


STEP 2: What You MUST Bring to the Clinic

·       Military Identification: A valid Common Access Card (CAC) or Uniformed Services ID card.

·       Printed PHAQ: The physical copy or confirmation number of your completed online Part 1 survey. 

·       Medical Devices: If you are required to use glasses, contact lenses, or hearing aids, you must bring them with you. You will undergo visual acuity and hearing screenings.

·       Immunization Records: Bring hard copies of any vaccine records for civilian-administered shots (like flu or COVID-19 vaccines) that have not been uploaded to your military tracking system. 

·       Medication Documentation: A comprehensive list or the physical bottles of all active prescriptions and over-the-counter supplements.

STEP 3: Appointment Preparation Checklist

Depending on your ordered voucher package, your PHA may include blood work, vitals tracking, and physical screenings:

1.    Fasting for Labs: If your voucher orders include routine blood panels (such as lipid or fasting glucose metrics), do not consume any food or caloric beverages for 12 to 14 hours before your appointment. Drinking plain water is allowed and encouraged.

2.    Avoid Alcohol: Avoid alcoholic beverages for at least 72 hours before checking in to prevent inaccurate lab anomalies.
