Important Information About Your Primary Care Provider Autonomous APRN Practice Disclosure Grandmother's Hands Family Health is an independently owned primary-care practice founded and operated by Dr. Carrollann Fernandes, DNP, APRN, CPNP, FNP-C, a Florida-licensed Advanced Practice Registered Nurse who is registered with the Florida Board of Nursing to practice as an Autonomous Advanced Practice Registered Nurse (Autonomous APRN). Under Florida law, an Autonomous APRN who meets the statutory requirements may provide autonomous primary care within the authorized scope of practice, including family medicine and general pediatrics. Autonomous practice means that the APRN provides care without a physician supervisory protocol or physician supervision, within the scope authorized by Florida law. Your Provider's Qualifications Provider: Dr. Carrollann Fernandes, DNP, APRN, CPNP, FNP-C Professional credentials: Dr. Fernandes has professional experience in pediatric and family primary care and provides primary-care services to children, adolescents, and adults within her authorized scope of practice. Nature of Autonomous Practice As an Autonomous APRN, Dr. Fernandes is authorized under Florida law to provide primary-care services within the scope of autonomous APRN practice, including family medicine and general pediatrics. Autonomous APRN practice does not mean that the provider practices outside the nursing profession. Rather, it is a legally recognized Florida APRN practice designation that permits a qualifying and registered APRN to provide specified primary-care services without a physician supervisory protocol. Appointments/Cancellations
With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators.
Emergencies
Our office provides on-call service for emergencies. Call 6165583199 so that the physician on call can be contacted.
If you are an established patient experiencing an acute medical issue that cannot wait until regular business hours, please call our main office line at [Your Phone Number] to be connected to our secure after-hours clinical triage system. Your message will be securely routed directly to our on-call autonomous provider.
Please note that official military readiness events and periodic weekend unit clinics operate under pre-authorized federal scheduling guidelines. For sudden cancellations or weekend logistics inquiries regarding an upcoming Guard/Reserve event, please log directly into your secure QTC portal or call our office coordinator.

For student-athletes and local youth leagues, we provide fast-track school, camp, and sports physicals for a flat, affordable $35 cash-pay rate. All mandatory Florida athletic clearance and school forms are fully executed on-site.
